How Will I Know When You Die?

I was surprised when I was visiting with a college friend in Seattle this fall.  She asked, “How will I know when you die?”  I was surprised with my response.  “I don’t know.”

When I returned home, I created a document that I put with my estate planning papers.  Here is what I created.  I encourage you to use this form or modify it to fit your needs.  Put it where someone you trust will be able to find it:

Family, friends and others to notify if I am seriously ill or die
